Palliative Care Social Work Team
The Palliative Care Social Work Team provides a social work and social care service for people affected by advanced progressive disease. It is usual for this team to be involved when needs become more complex. The team employs care managers who work either on their own or with others to help people benefit from social care services. This process is referred to as 'Assessment and Care management'.
Care provided by the Specialist Social Care Team is subject to an assessment. In completing and assessment the care manager will discuss their needs with the service user. The care manager will provide service users with information about Social Services and the speciality. Once needs are identified the care manager will agree a care plan and make arrangements to implement services and any other assistance required as part of the care plan. A typical plan will include aspect of practical, financial and psycho-social support.
